The Intake Child or Youth page allows an agency worker to add a child or youth into the Placements module. After an intake is complete, records can be viewed on the Children & Youth page.
What information is required to complete an intake?
In order to intake a child or youth, only first and last name are required. However, to ensure best results with Binti's Placement Search feature, we highly recommend entering a date of birth and zip code (community of origin) if possible.
What information is collected on Intake Child or Youth?
The Intake Child or Youth page is a comprehensive intake page that collects a wide range of information about a child or youth. Each section of the page is outlined below.
Child Section
The child section collects basic demographic information including the following:
- Name
- Date of Birth
- Gender
- Primary language
- Zip Code (Community of Origin)
- Social Security Number
- Medicaid Number
- Comments (Internal - Not Shared)
Additional Gender and Sexual Orientation Information Section
The child section collects information including the following:
- What sex was assigned on their birth certificate? (Internal - Not Shared)
- Gender expression
- Further information about gender and sex
- Sexual orientation
For gender expression and sexual orientation questions, there are options to indicate that the child or youth prefers not to answer or has not been asked because they are too young or for other reasons.
Ethnicity and ICWA Information Section
The child section collects information including the following:
- Ethnicity
- Federally Recognized Tribal enrollment
Contact Information Section
The child section collects information including the following:
- Address (home of origin and/or placement address)
- Phone number
Sibling Information Section
The child section collects information including the following:
- Checkbox to indicate child has no siblings
- Associated siblings
In order to associate siblings, records for all siblings must already exist in Binti. If records for every siblings do not yet exist in Binti, follow these steps:
- Complete the intake process for each sibling you would like to associate.
- Once records for all siblings exist, navigate to the Children & Youth dashboard.
- Find the child you would like to associate siblings for.
- Click on Actions > Edit Child
- Enter the names of any associated siblings in the Sibling Information section
School Section
The child section collects information including the following:
- School enrollment status
- Eligibility to attend non-public school
- Current school attended
- Current grade enrolled
- Further education information
Placement Considerations Section
The Placement Considerations section collects information including the following:
- Visitation schedule
- Strengths
- Case Carrying Worker Placement Recommendations (Internal - Not shared)
- Characteristics, Medical Conditions, and Behaviors
- Can be in home with pets?
- Parental rights terminated?
- Pregnant or has a child?
- Other Youth Needs
Characteristics, conditions, and behaviors input in this section are matched with caregiver placement preferences in order to facilitate the best placement.
Assigned Level of Care Section
The Assigned Level of Care section collects information including the following:
- Level of Care
- Effective date
Level of Care is customizable per agency and can be modified by reaching out to your Customer Success Manager.
Agency Section
The Agency section collects information including the following:
- Referring Agency (if applicable)
- Ability to indicate a child was never placed at the agency
- Client ID
- Date Child Joined Agency
- Permanency/Case Carrying Worker Assigned to Child
- Checkbox to enable/disable Requirements For This Child (Case Management only)
- Additional Worker Contact Info (Internal - Not Shared)
The option to mark a child as "Not in agency care" can be used to document a referral where the agency did not ultimately provide care. It will move a child or youth to the "Not in Agency Care" tab on the Children & Youth dashboard.
