As an applicant or provider how do I go back to a previously approved application to view forms and/or docs.

Overview

This article is for applicants. Forms and supporting documents from different approval cycles can be accessed via applicant portal by clicking on your name on the left side of the portal.

 

To access documents and forms from a previously approved application

  1. Click on your name on the left side of the portal to access a drop down menu.
  2. If you have previously approved applications you will see a link(s) to them there.
  3. Choose the link to the application you wish to access.
  4. Continue until you have reached the desired approval cycle.
